I Lost My Certificate and Need a Copy (click to find out how to get a copy) PLEASE NOTE: The 2008 Procedural Guidelines for MHRT/C certification have been updated to reflect approved courses and degree programs for Vocational Aspects of Disability. This list of approved courses and programs is effective as of January 1, 2009. Please see Appendix A, Appendix B, and Appendix C for details. With the implementation of this requirement, it is understandable that students who complete their final course(s) or degree for full MHRT/C certification in December of 2008 have faced some challenges in submitting documentation (transcripts, degrees, etc) to the Muskie School’s Center for Learning by the January 1st deadline. Recognizing the challenge of submitting paperwork by the January 1st deadline, OAMHS has made the following decision: All students who complete their final MHRT/C course(s) or degree from an approved academic provider in December 2008 will be given an extension until February 1, 2009 to submit documentation (copies of official transcripts, diplomas, etc) of their completed course(s) to the Center for Learning for full MHRT/C certification. As a result, students should have sufficient time to access copies of their transcripts or diplomas that verify their completion of the course(s)/degree for MHRT/C certification. Please note that this extension to submit paperwork applies only to those who are completing their final course(s) or degree required for full MHRT/C certification in December 2008. Other extensions will not be granted. HOW TO REQUEST A NEW COPY OF YOUR LOST CERTIFICATE If you have misplaced/lost your MHRT certificate, you may request a copy of your certificate from the Center for Learning, UNLESS your certificate was issued by one of the following:

For all others, you may submit your request for a new copy of your MHRT certificate in writing to the Center for Learning.
