I Lost My Certificate and Need a Copy (click to find out how to get a copy)

PLEASE NOTE:  Effective November 1, 2004, there are now two levels of Provisional MHRT/Community certification:  Level A and Level B.  Level B certification is the same as the Provisional MHRT/C certification prior to November 1st.  Level A is for applicants with a human services-related degree.  For full details, please see the letter from Christine Robinson, Adult Mental Health Services, Department of Health and Human Services and attached list of acceptable degrees for this new level of certification in your choice of Word or Adobe formats.  We will update the online guidelines shortly to reflect these changes.

Our MHRT/Community Non-Academic Course Standards and Trainer Application form are now available for download in Adobe Acrobat format.

By request, we have created a an MHRT/Community work experience waiver request form you can fill out and submit to your supervisor when requesting a course to be waived through your work experience.  You can download the form in:  Word or  Adobe  formats.

Classes/Trainings towards MHRT I or MHRT/C certification:
 
At this point in time, the Center for Learning does not offer any of the courses/training required for certification.  We only oversee the certification programs.  Please see either the MHRT I Resources page or Appendix A in the Community Guidelines for where you can take the necessary courses/trainings.

PLEASE NOTE:  Effective February 1, 2004, you may substitute a Full MHRT/Community (or II or III) certificate in place of the Mental Health Support Specialist (MHSS formerly RCS) course when you are applying for MHRT I certification.  All other requirements for MHRT I remain unchanged.

Before sending the Center for Learning an email with your question in regards to certification, please be sure to read both the MHRT/Community Guidelines and the MHRT/Community FAQ's.  These two documents will answer most questions.

 

HOW TO REQUEST A NEW COPY OF YOUR LOST CERTIFICATE

If you have misplaced/lost your MHRT certificate, you may request a copy of your certificate from the Center for Learning, UNLESS your certificate was issued by one of the following:

  • The University of Maine at Augusta
  • The University of Maine at Farmington
  • University of Southern Maine's Graduate School MHRT program

If your certificate was issued by one of the above, you need to contact that institute directly.

For everyone else, you may submit your request in writing to the Center for Learning.  We can only release a copy of your records directly to you and not to any other party.  In the request we need the following:

  • Your name as it appears on the certificate
  • The last four digits of your SSN
  • The approximate date you received your certificate
  • The address or fax number to send the copy to at your attention
