Data Reminders

• If a student has been seen at the health center, keep them registered until after the export at the end of the school year.

• Document the date of students' last physical exam.

• When a student completes the Health Risk Assessment (HRA) questionnaire, use procedure code CPT 99420. If they answer both depression questions, use Maine procedure code 5902M for PHQ2 screening.

• If a student is at-risk according to their HRA, provide treatment/counseling. Document baseline Stage of Change. Re-evaluate Stage of Change at least once during the school year.

• Use the treatment/counseling codes provided on the Quick Reference Sheet for: smoking, nutrition, physical activity, alcohol/drug use.

• Make sure the student has had treatment/ counseling before assessing Stage of Change.
