Summary of Project: Vermont Department of Children and Families, Family Services Division's January 2008 Transformation Plan targets many structural and practice changes with the overarching goal of increased engagement with families and earlier intervention. The NCIC project is the next phase of the work that has already been completed toward that goal. The project will include developing practice guidance, to provide assistance and support to each district office to implement the Practice Model and the social work process guidance. A hiring protocol and performance evaluation process will also be developed that support the skills, values and beliefs that are the foundation of the practice model and guidance. There will also be a focus on external stakeholders to align contracted services with the practice model. In the area of data, access to meaningful data will be improved, the use of data among management and supervisors will be addressed and data will be collected on contracted services to assess outcomes and model fidelity.
