Informed Approval --------

The person or guardian must be informed of the privacy practices of the provider, the laws and rules protecting confidentiality of health information, and the exceptions which allow disclosure without the person’s approval. 

Information should be presented as simply as possible in whatever form of language or communication the person understands best (e.g., large type, audiotape, Braille, ASL, etc). Braille or audiotape version of Rights of Recipients of Mental Health Services can be requested from the ADA/EEO officer in Central Office.
